PHYSICALS

If your son is in 7th - 12th grade, he MUST have a physical to tryout.  Below are the links to the forms needed in order to participate in our program.  These forms must be completed and handed into the Athletic Office prior to the first day of tryouts.  DO NOT HAND THEM TO A COACH!  THEY ARE NOT ALLOWED TO TAKE THEM!
